You received the First Day Packets that were sent home last week.
We ask that you review and sign all the yearly, required documentation as follows and have your child/children return their forms directly to their teachers by this Thursday, September 6th.
Green Enrollment Form
Must be signed and dated
If families have moved, we do need Proof of Address in the form of a September utility bill in the name of a parent/guardian.
Meal Application
This form must be filled out by every student even if you may not qualify as this application is tied to our State funding and in turn our school site. We appreciate you filling this out, signing and dating it.
Gold Health Card
This must be filled out for every child for the Health Office to have on file.
Universal Form
This is an important document for you to designate permissions. Also, if you want your child’s picture(s) in the McKinley Yearbook for 18-19, do check the yearbook permission or if you want to Opt out.
